ACCIDENT REPORTING FORMS
An Accident Reporting Form should be placed in each vehicle driven on University business. These forms are available through Risk Management & Safety by calling (574) 631-5037. A copy of the form is available on this site.
AUTOMOBILE INSURANCE IDENTIFICATION CARDS
Many states require proof of financial responsibility in case of an accident. An Automobile Insurance ID Card should be placed in each vehicle driven on University business. These cards are available through Risk Management & Safety by calling (574) 631-5037.
CERTIFICATES OF INSURANCE
Car Rental Agencies, or others, may require verification of insurance coverage. Certificates of Insurance can be obtained through Risk Management & Safety by calling (574) 631-5037. A completed request form can be FAXED to (574) 631-8794 or sent via e-mail to Davey.1@nd.edu.

Defensive driving courses are offered through Transportation Services. All persons driving vehicles on University business should complete a defensive driving course every three years. Classes can be scheduled by calling Transportation Services at 631-6467 or email: undtrans@nd.edu
MOTOR VEHICLE RECORD CHECKS
A motor vehicle record should be obtained for all persons driving University owned, hired or non-owned vehicles. Motor vehicle records may be obtained through Risk Management & Safety. A completed request form can be FAXED to (574) 631-8794 or sent via e-mail to hafner.4@nd.edu.
NON-OWNED AND BORROWED VEHICLE COVERAGE ON UNIVERSITY BUSINESS
LIABILITY
The University's automobile policy will provide excess liability coverage when the non-owned, hired, or borrowed vehicle is driven by an authorized person on University business.
PHYSICAL DAMAGE
This section provides basic coverage information regarding the University's non-owned vehicle insurance coverage. It should not be relied on for policy interpretation. Any specific questions should be referred to Robert M. Zerr, Director, or Barbara Davey, Risk Management Specialist, of the Risk Management and Safety Department, (574) 631-5037.
UNIVERSITY OWNED, RENTED, AND LEASED VEHICLE COVERAGE
LIABILITY
The University maintains automobile bodily injury and property damage liability coverage for all owned vehicles. Primary coverage is extended to all University personnel while using a University vehicle with permission.
PHYSICAL DAMAGE
Physical Damage coverage is provided for all University owned, rented and leased vehicles. A $500 deductible applies for comprehensive and collision claims. Collision is defined as colliding with another object, including overturn. Comprehensive is defined as any cause of loss other than collision or overturn, including fire, vandalism, windshield break/crack or hail.
