Academic Programs for Minors
Faculty/Staff planning an Academic Program for Minors should complete the Academic Program for Minors Application and submit it to Barbara Davey in the Risk Management & Safety Department. Once the application is reviewed and accepted, our office will provide
- “Camps on Campus” – A Risk Management & Safety Bulletin
- Waiver, Release and Indemnification Agreement to be signed by the parent or legal guardian of a minor child
- Health Information and Consent for Treatment Form
- Student Accident Insurance for Participants
